Deviation management software: six vendors read against the deviation record

Published , reviewed

Verdict

Best overall
TrackWise DigitalQuality events, deviations and CAPA run in one system, and its Part 11 and Annex 11 assessment (dated March 2019) describes a trail that records user ID, old and new value and time stamp, with entries that cannot be deleted.
Best for a small lab
QualioDeviations live in its events module beside CAPA and change control, audit trail records are kept for the lifetime of the system, and customers receive a validation pack with test scripts and a Part 11 assessment.
Best across sites and partners
Veeva QMSIt runs deviations, lab investigations, CAPAs and change controls in one Vault application built to bring sites, suppliers and contract manufacturers into one system, and its help pages describe each eSignature as a read-only record of name, title, date and time and meaning.
Chromolithograph of an open records drawer with one file marked by an amber tabFig. 01
One file pulled and tagged: a deviation stays open until someone signs for what it touched.

A deviation file shows an inspector whether a site noticed what went wrong, stopped it spreading and understood it. For a drug maker the duty is two sentences long: any deviation from written procedures “shall be recorded and justified” § 211.100(b), and any unexplained discrepancy “shall be thoroughly investigated, whether or not the batch has already been distributed” § 211.192. Kept in software, that record and the QA signatures on it are Part 11 records.

This page follows one deviation from detection to closure, then reads six quality systems against the 9 Part 11 rows the record leans on. The deviation rows our register cannot rate close the page as a demo script. What happens after the investigation is on the CAPA software comparison; the wider suite is on the Part 11 eQMS comparison.

What the rules ask of a deviation

Part 11 never says which records to keep. For a deviation the predicate rule is Part 211, and it asks for four things: a record made at the time, a justification, an investigation that “shall extend to other batches of the same drug product and other drug products” that may be associated, and “A written record of the investigation” that includes “the conclusions and followup” § 211.192. The quality control unit holds the authority to see that errors “have been fully investigated” § 211.22(a), and each year the product review takes in the investigations made under § 211.192 § 211.180(e)(2).

A failed lab result runs through the same section. FDA's OOS guidance (Revision 1, May 2022) reads § 211.192 as requiring an investigation “whenever an OOS test result is obtained”: a Phase I laboratory investigation first, then a full-scale investigation when no laboratory error is found. ICH Q10 then treats deviations as one input to the CAPA system § 3.2.2 and to the monitoring of process performance and product quality § 3.2.1.

Outside drug CGMP the words change, not the record. A GLP lab's deviations from SOPs are “authorized by the study director and … documented in the raw data” § 58.81. Device makers work under the QMSR, which incorporates ISO 13485 by reference, and often file the same events as nonconformances (our QMSR guide covers the switch). In the EU, a failure of the computerised system itself is also an incident under Annex 11 § 13.

One deviation, detection to closure

Pick a worked example. Each stage shows what the record holds, the signature it carries where there is one, the rule behind it and the Part 11 control that applies. The process example starts where most deviations do, on the batch record; the electronic batch record comparison reads the systems that raise them.

Worked example (illustrative values)

Blend time over the batch record range

DEV-2026-0412, example tablets 10 mg, lot 26A114

  1. 01Detect

Granulation operator, line 2, 2026-10-13 09:40: blend time 14 min against 10 to 12 min in step 7.3 of the master batch record. Entered on the batch record at the time, not at the end of the shift.

Rule: Procedures are “documented at the time of performance” and any deviation “recorded and justified” § 211.100(b)

Part 11: The entry that creates the record is trailed with date and time § 11.10(e)

  2. 02Contain

Lot 26A114 placed on QA hold; the blend moved to quarantine. Line 2 runs on only after the timer is checked against a stopwatch.

Rule: The quality control unit has the authority to approve or reject in-process materials and drug products § 211.22(a)

Part 11: The system enforces the order of steps, so a held lot cannot be released around the record § 11.10(f)

  3. 03Classify

Major: a process parameter outside its validated range. The grade comes from the site's deviation SOP.

Part 11: A change of grade is trailed and the old grade stays readable § 11.10(e)

  4. 04Assess impact

Product: extra blend uniformity and dissolution samples. Data: none affected. Other batches: lots 26A109 to 26A113 ran on the same timer, and each batch record is reviewed.

Rule: The investigation “shall extend to other batches of the same drug product and other drug products” that may be associated § 211.192

Part 11: Only authorised roles can mark a batch or a data set as not affected § 11.10(g)

  5. 05Investigate

Root cause: the blender timer was replaced on 2026-10-06 and never added to the calibration schedule. The first root cause, entered as operator error, was corrected on 2026-10-15 with a reason; both entries stay readable in the trail.

Rule: “A written record of the investigation shall be made and shall include the conclusions and followup” § 211.192

Part 11: “Record changes shall not obscure previously recorded information” § 11.10(e); the reason for a change is documented Annex 11 § 9

  6. 06Escalate

CAPA opened and linked for the calibration schedule gap. A change control for the timer swap is opened after the fact and linked. No OOS: the extra results are within specification.

Rule: CAPA should follow from the investigation of deviations, among other sources ICH Q10 § 3.2.2

Part 11: Adding or removing a link to a CAPA or change control is itself a trailed edit § 11.10(e)

  7. 07Approve and close

QA approves the investigation and the disposition of lot 26A114. The CAPA stays open on its own effectiveness check.

Rule: The annual product review covers “investigations conducted under § 211.192 for each drug product” § 211.180(e)(2)

Part 11: Accurate and complete copies, and retrieval for the whole retention period § 11.10(b), (c)

A role stands in for the printed name a real signature must show. The stage list is ours; the rules are quoted.

Inspection risk

A deviation closed as operator error with retraining, and no look at other lots. § 211.192 extends the investigation to other batches that may be associated, and the annual review is where the same root cause, filed five times, shows up.

The two Part 11 controls that matter

FDA does not intend to enforce the Part 11 version of the audit trail, its 2003 guidance says, but the signature sections still apply in full, as does every predicate rule, and the same guidance names exactly the case a deviation is: audit trails are “particularly appropriate when users are expected to create, modify, or delete regulated records during normal operation.” An investigation is edited for days. Two controls carry the weight.

The audit trail on the investigation

Part 11 asks for a “secure, computer-generated, time-stamped” trail of every entry that creates, modifies or deletes the record, and “Record changes shall not obscure previously recorded information” § 11.10(e). On a deviation, the edits an inspector looks for are a grade lowered after QA review, a root cause rewritten after approval, an other-batches field cleared, a CAPA link removed and a due date moved. Annex 11 adds that the reason for each change is documented and the trail “regularly reviewed” Annex 11 § 9.

The trail is also part of what QA approves. FDA's 2018 data integrity Q&A says production and control records, “which includes audit trails, must be reviewed and approved by the quality unit” under § 211.192. The audit trail review checklist builds that review for one system, and what an audit trail must capture covers the fields.

Signatures with meaning on QA approval

A deviation usually carries more than one signature: QA's review of the grade, the approval of the investigation, the closure. Each must show the printed name, the date and time, and “the meaning (such as review, approval, responsibility, or authorship)” § 11.50(a), on screen and on every printout § 11.50(b). The meaning is what tells the reader which decision was signed. The signature is linked so it cannot be moved to another record § 11.70, needs two components such as an ID and a password § 11.200(a), and only the roles you authorise can sign § 11.10(g). Test one approval block with the signature manifestation check.

An approval on text that changed

If an investigation can be edited after its approval and the approval still shows against the new text, the signature now vouches for words its signer never read. Ask how the system handles it: re-approval, or a visible flag.

Requirement by requirement

The rows run in the order a deviation meets them: the trail on the investigation first, then the QA signature, then access, copies and validation. The open-system row (§ 11.30) depends on how your system is hosted and reached, not on the deviation workflow, so it sits on the eQMS comparison.

Deviations, events and what sits beside them

Search a vendor's site for “deviation” and you may miss the module. Qualio files deviations under events, and other suites name quality events. What matters more is what sits in the same system, because the links a deviation needs (to the CAPA, the change control, the lab investigation) are easiest to keep when both ends live in one place.

Lab investigations are named beside deviations in the description of Veeva QMS. If your OOS work lives in a LIMS instead, the link between the OOS record and the deviation crosses systems, and both ends need their own trail; the LIMS comparison reads that side.

On the pages we read, the audit trail is described by Veeva QMS, TrackWise Digital, MasterControl Quality Excellence, Scilife and Qualio, and what a signature shows is described by Veeva QMS, TrackWise Digital and MasterControl Quality Excellence. The notes in the table are the vendors' own details; a cell without a note is a claim or a description with nothing more to add.

TrackWise Digital takes best overall because deviations, quality events and CAPA run in one system and its assessment describes the trail down to old and new values, with the date caveat above. Qualio suits a small lab: events sit beside CAPA and change control, and the validation pack covers what a small team cannot write itself. Veeva QMS fits a company whose deviations happen at more than one site or at a contract manufacturer, with lab investigations in the same application. These match our CAPA verdict for TrackWise Digital and Qualio; the third use case differs because a deviation, unlike a CAPA, often starts at someone else's site.

The deviation rows to test in a demo

The rows that make deviation software different are not in our register: we read no vendor's pages on them, so they carry no status. Run them in every demo, copy them into your user requirement specification, and test them again in validation.

Demo script, 9 steps

  1. 01

Open a deviation as an operator would, with the event time an hour earlier than now.

A pass: The system stamps its own entry time and keeps the event time as a separate field.

    § 211.100(b); § 11.10(e)

  2. 02

With a deviation open against a lot, try to release the lot or close its batch record.

A pass: Release is blocked or flagged until QA has approved the deviation.

    § 211.192; § 11.10(f)

  3. 03

After QA has reviewed the grade, change it from major to minor.

A pass: The trail shows old and new grade, who, when and why, and the earlier QA review is shown as made before the change.

    § 11.10(e); Annex 11 § 9

  4. 04

Fill in the impact on other batches.

A pass: Other lots are entered as lot numbers that link to their records, not as free text or N/A.

    § 211.192

  5. 05

Rewrite the root cause after the investigation has been approved.

A pass: The first root cause stays readable, and the approval is flagged as made on the earlier text.

    § 11.10(e); § 11.70

  6. 06

Take an OOS result from Phase I to a full-scale investigation.

A pass: The OOS record and the deviation link both ways, and the original result stays in the record after a retest.

    FDA OOS guidance (2022); § 211.192

  7. 07

Open a CAPA and a change control from the deviation, then close the deviation.

A pass: Both links show from each side, and closing the deviation does not close the CAPA.

    ICH Q10 § 3.2.2

  8. 08

Sign the closure as a user without a QA approval role, then as QA, and print it.

A pass: Only the QA role can sign, and the screen and the PDF both show name, date and time, and meaning.

    § 11.10(g); § 11.50

  9. 09

Run a year of deviations by product and root-cause code and export it.

A pass: The report reads the live records, the counts match them, and the export is complete and readable.

    § 211.180(e)(2); § 11.10(b)

When the investigation ends in an action, the CAPA form template lays out the record that follows. Changes to equipment or procedures go through change control.

How we read the vendors

Verdicts are editorial: no vendor can buy a verdict position or displace another vendor.

verified means we read the control described on the vendor's own documentation (help centre, validation pack page, compliance paper or trust page) on 2026-10-22. stated means the vendor claims it and we did not check it. not stated means the vendor publishes nothing on it.

FDA keeps no list of certified or approved Part 11 software. Compliance is a validation claim each regulated company makes for its own use of a system, so a vendor's Part 11 claim is where your validation starts, not where it ends.

Questions

Does FDA require deviation management software?

No. Part 211 requires the record: deviations “recorded and justified” § 211.100(b) and a written record of each investigation § 211.192. It does not say how. Once that record is kept and relied on electronically it is a Part 11 record, and FDA keeps no list of certified software, so the system is validated by you for your own use § 11.10(a).

What is the difference between a deviation, an OOS result and a CAPA?

A deviation is a departure from a written procedure § 211.100(b). An OOS result, in FDA's guidance, is a test result outside the specifications or acceptance criteria set in an application, a drug master file, an official compendium or by the manufacturer, including in-process tests. Both are investigated under § 211.192. A CAPA is the action that follows an investigation; ICH Q10 lists deviations among the sources a CAPA system draws on. Our CAPA software comparison reads that side of the record.

Does every deviation need a CAPA?

No rule says so word for word. § 211.192 asks for conclusions and follow-up, FDA's OOS guidance says the need for corrective and preventive action is “implicit” in that duty to investigate, and ICH Q10 sets the effort “commensurate with the level of risk”. So the deviation record should either link a CAPA or say why none was opened, and the closure signature covers that decision.

Does a planned deviation still need a deviation record?

Yes. § 211.100(b) covers “any deviation from the written procedures” and makes no exception for one approved in advance. The advance approval is the justification; the record of what was actually done is still made at the time.

How long do deviation records have to be kept?

Part 11 sets no period of its own; the predicate rule does, and the audit trail is kept at least as long as the record § 11.10(e). For a drug maker, records tied to a batch are kept at least 1 year after the batch's expiry date § 211.180. In the EU, batch documentation is kept one year after expiry or at least five years after certification by the Qualified Person, whichever is longer Chapter 4.11.

Who has to sign a deviation, and can it be done electronically?

The quality control unit reviews and approves the production and control records § 211.192 and has the authority to see that errors “have been fully investigated” § 211.22(a); your SOP names who else signs. Electronic signatures work if each one shows printed name, date and time, and meaning § 11.50, and FDA's 2003 guidance says it enforces the signature sections in full. The signature manifestation check tests one approval block.

Can we run deviations in a spreadsheet?

Only with the same controls: a trail on every change that keeps the old value, signatures linked to the record, and access by role. A shared workbook rarely gives you those without add-ons. See Excel and Part 11 before you rely on one.
